- NISS (Social Security Identification Number)
- Cartão de Cidadão (Citizen Card) or Chave Móvel Digital (for online application)
- Be covered by a Social Security scheme in Portugal
Step by step
Go to Segurança Social Direta at www.seg-social.pt
Sign in with CMD or NIF + password
In the menu, select 'Documentos e Formulários' (Documents and Forms)
Click 'Cartão Europeu de Seguro de Doença' (European Health Insurance Card)
Confirm your details and delivery address
Submit the request — it is completely free
The card is sent to your address within 5 to 7 business days
The card is valid for 3 years from the date of issue
