Inscrição no Centro de Saúde (Health Centre Registration)
Register at your local Health Centre to get access to a family doctor, appointments, vaccinations and other primary healthcare services.
What you need before you start
- Cartão de Cidadão (Citizen Card) or valid identification document
- Health User Number (if you already have one — otherwise it is created during registration)
- Proof of address (water bill, electricity bill or rental contract)
- NIF (Tax Identification Number)
Step by step
Identify the Health Centre (UCSP/USF) in your area of residence
Go to the Health Centre with the required documents
At the reception desk, request registration as a patient
Fill in the registration form with your personal details
You will be assigned a Health User Number (if it is your first time)
You will be placed on the waiting list for a family doctor (if no immediate vacancies)
Through SNS 24 you can check if a family doctor has been assigned to you
